Using the Network’s Vascular Access Report to Help in the Management of Your Facility’s Long-Term Catheter Reduction Efforts

Qsource ESRD Network 12 is happy to announce that we can resume distribution of facility-specific feedback reports in support of your management of vascular access submissions through CROWNWeb. Every Medicare-approved dialysis facility should have received a Vascular Access Report recently.  We strive to send out this report before the end of month to give your facility the chance to review, add, or change data before everything is locked down.  You may also use this report to help manage your efforts in reducing long-term catheters. If you have any questions or did not receive your Vascular Access Report, please email NW12-QIDept@qsource.org

Staff will be able to answer questions from participants regarding barriers or opportunities to improve relationships with dialysis centers.
