Improving Outcomes through Patient Engagement

When patient are active in their own health care they have better outcomes.

This year, the quality improvement activities have included a patient partnership component and the selection of Centers for Medicare and Medicaid Services (CMS) driven patient engagement objectives to incorporate into facility improvement activities.
