2020 Quality Improvement Activities (QIAs) Resources and Reporting Links

Welcome to the 2020 QIA Landing Page. We hope you will find this page useful as a “one-stop-shop” for all resources, information, and reporting links related to the quality improvement activities occurring throughout Qsource ESRD Network 12.

All QIAs were launched with the Network Council Webinar, held January 8, 2020.  A recording of this webinar is available here.  We ask that all units have someone review this information as it serves as a great orientation to Network and the QIAs.

Each facility is requested to:

  1. submit an evaluation of the Network
  2. review the Participation Commitment
  3. submit an attestation.
