Do you know of a patient or family member that would be interested in helping to guide the Network to improve the patient experience in region?
As part of the Network’s governing body, the Patient Advisory Council provides the patient perspective in our patient education and QIA planning, implementation and evaluation. We are especially in need patients who are currently doing dialysis at home and for family members/care partners to join the PAC.
If you have a patient or family member of a patient who may be interested in becoming a member of our Patient Advisory Council, we encourage you to have them complete the PAC application. We are accepting applications for the 2020 contract year.
Please submit applications by Aug. 30 so we can include “new” members in our Annual PAC Summit held in October. For more information or to recommend a current Network Patient Representative, please contact DeeDee Velasquez-Peralta at 816-880-9990.
