This month’s “Be Active in your Care” patient engagement topic is “My Plan: My Wishes” and the Patient Engagement Action for staff to encourage is: Let staff and family know what you want for your care.
When patients are actively involved in the development of their plan of care the goals can be tailored to meet their individualized needs and can lead to better outcomes and quality of life. The resources compiled in the Topic of the Month sheet are focused on making sure patients are active in developing their plan of care by sharing their goals as well as communicating their wishes for care now and in the future through advance care planning tools. The Network encourages all facilities to work with their Quality Assessment and Performance Improvement (QAPI) team to identify strategies to increase patient engagement in their plan of care by offering Plan of Care (POC) meetings off the treatment floor and/or opportunities to participate via phone or virtually.
